  • Cefazolin and Probenecid - May 14, 2015

Changes to Order Sets Effective May 14, 2015

1. Cefazolin (Ancef) changed to 2gm IV (previously there were some OS with 1gm and others with 2gm), as per ID and best practice recommendations.
2. Probenecid orders have been aligned in the order sets so they are consistent. Any ED OS with Probenecid should now be consistent - 1000 mg PO Q24H and always be included with Ancef.  It is now back in stock in the ED and will be for the foreseeable future.
3. If you are starting using Cefazolin (Ancef) and arranging HPTP (for >8-10H, we are strongly encouraging the use of *Probenecid* *1000mg PO 15-30 minutes before Cefazolin *to avoid patients having to return to the ED for repeat Cefazolin dosing. Looking at the number of times Probenecid has been ordered over the last 6 months, it appears as though we are greatly
under-utilizing this treatment option, and requiring patients to return to the ED unnecessarily (cost to the system and inconvenience to the patient).
4. * I highly recommend you using the specific order set *(i.e. ED cellulitis) rather ordering these meds off the ED Common Meds. This will ensure that you are giving the right dose for the specific condition. This is less relevant for this situation but will be more relevant for future changes (such as the upcoming Ceftriaxone dose changes, where the dose of drug is dependent on the condition).
