Sunridge Multidisciplinary Teams

The following allied health team members are available at our clinic to help you in your care journey. Patients must be 18 years or older to see a member of the allied health team. Speak to your family doctor if you think you'd like to see one of the below:

 

  1. Mental Health Therapist: Addressing most mental health concerns of adult patients: chronic pain, depression, anxiety, insomnia, Post-Traumatic Stress Disorder.  Teaches stress reduction skills and relaxation techniques; helping with changing problematic behaviors, such as tobacco, alcohol or drug use and assisting with weight management.
     
  2. Mental Health Therapist: Solution-focused, strength-based counselling for adults with mild to moderate mental health concerns including depression, anxiety, stress, and other issues we all struggle with at times related to the human condition.  Focusing on wellness through developing insight and self-awareness, effective goal-setting, and adopting healthy behaviors. 

     
  3. Social Worker: Counselling in the areas of grief and loss, mild-moderate mental health concerns, illness adjustment, crisis intervention and life transitions.  Resource support, advocacy and application assistance.  General need support or referrals in the areas of finances, legal, senior’s issues, accommodation, medication coverage, transportation, mental health/addiction, career/education, domestic violence, homecare/caregiver, and advanced care planning. 
     
  4. Diabetes Educator: Diabetes support and counselling for patients with Type I diabetes, Type II on two or more oral hypoglycemic agents, insulin starts, recurrent or severe hypoglycemia/hyperglycemia. 
     
  5. Dietitian:  Nutrition education and counselling to clients with the following conditions: Pre-diabetes/diabetes, dyslipidemia, hypertension, weight management, food allergies/ intolerance, gastrointestinal diseases (Celiac Disease, IBD, GERD) IBS, malnutrition, nutrient deficiencies, unexplained weight loss, liver disease, prenatal nutrition issues.

Appointment & Clinic Policies

What you need to know as a patient at Sunridge Family Medicine Teaching Centre

When calling for an appointment, please tell reception the reason for your visit and have your Alberta Health Care number ready.

Family members are welcomed to join if required as a caregiver. If the family member needs to be seen by a doctor, they must make a separate appointment. 

 

Please check in with reception upon arrival. They will update your information and inform the staff you've arrived. This information is required for every in person clinic appointment:

  • Alberta Health Care card (MUST present this card)
  • Drivers Licence, current address
  • Your phone number
  • Emergency contact number
  • Do you consider your Physician at Sunridge Family Medicine Teaching Centre to be your only family Physician?

Call at least 24 hours in advance if you cannot make your appointment. This frees up your visit time for another patient.

  • Call our main phone number at 403-943-5000, press option 3 and leave a message to cancel your appointment.
  • Please provide 24 hours when cancelling your appointment.

If you’re going to be late for your appointment please let us know. In some cases there may be a delay or you may be asked to reschedule your appointment.

  • If you are over 10 minutes late you MAY be asked to re-schedule your appointment
  • If you are over 30 minutes late you WILL be asked to re-schedule your appointment

If multiple appointments are missed without cancellation, you may be asked to seek medical attention at another facility. If you are a new patient and you miss your first appointment, another appointment will not be made with any physician in the clinic in the future.

If a patient has not been seen in the clinic for more than three years, the patient’s chart will become inactive and letter sent to the patient explaining this by the Primary Care Coordinator. 

Prescription Refills

Re­fills are not routinely done by phone. Please partner with your physician and pharmacist to avoid running out of medication between visits.

  1. You may contact your drugstore for the status of re­fills that may be left on your prescription.
  2. If there are no refills left, call the clinic early to arrange an appointment to review your condition and medications.
  3. Your community pharmacist can extend most prescriptions for a short time to last until your next appointment. Exceptions are narcotics, triplicate order medications, antibiotics and steroids.
  4. Please bring all your medications in the original containers to your visits at the clinic. This will assist greatly in your conversation with your Physician regarding the amount of medication being ordered, and the number of refills before your next visit. Book the next visit before leaving the clinic, to ensure that the timing of your next visit will prevent running out of medication.

Referrals & Special Tests

You must see your family physician to get a referral to a specialist. Referrals will be processed by the referral staff. You will be called about the date and time when it is con­firmed. If you are not able to attend the appointment time or date given, please call the specialist’s of­fice to re-book.

Test Results

Please do not assume “no news is good news”. When tests are being ordered, partner with your physician in planning what follow-up is needed regarding results. This could result in another visit – if so, book that next visit before leaving the clinic. Your physician can advise how long results would usually take to arrive, and can suggest when the best timing for a follow-up visit would be.

Uninsured Services

Certain services are not covered by Alberta Health and a fee will be charged for these services. The exact amount will depend on the service. Speak with reception for the amount. If you have financial concerns, please discuss these charges with your doctor.
