Why We Ask You to Fill Questionnaires

By sharing your experience, you help guide your recovery, shape your care plan, and contribute to better care for others in the future. There are no right or wrong answers.

We use these questionnaires to understand different parts of your health and recovery. Each one focuses on something unique, such as:

  • Pain – How much pain you feel and how it affects daily life
  • Mood and feelings – How you feel emotionally, including stress or worry
  • Confidence – How confident you feel managing your pain and daily tasks
  • Thoughts about pain – How your thoughts and feelings affect your experience of pain
  • Social needs – Factors that might make it harder to stay healthy, like housing, money, or support at home
  • Quality of life – How you are doing overall, including health, happiness, and energy
  • Fatigue – How tired you feel and how it affects your life
  • Mobility and function – How pain affects movement and everyday activities
  • Life satisfaction – How satisfied you feel with your life overall
  • Life after brain injury – How a past brain injury affects your daily life
  • Recovery risks – Factors that might slow your recovery

Putting all this information together helps us see the whole picture of your health. It allows us to personalize your care, track your progress, and adjust your treatment plan over time.

Please note: Your care team may ask you to complete a few other questionnaires depending on the program you are part of.

  • At the start of your program
  • Partway through your program
  • Near the end of your program
  • You will get a secure website link and access code by email to access your questionnaires
  • You can fill them out online using a computer, tablet, or smartphone
    • If online completion isn’t possible, a member of our team can help
  • Depending on the combination of questionnaires being asked, it may take about 10–30 minutes to complete
    • If you’re unable to finish the set of questionnaires in one sitting, you can use the “Save and Return Later” feature and follow the instructions on the screen.

  • PHQ-4 – screens for mood, anxiety, and emotional well-being
  • PSEQ – looks at your confidence in managing pain and daily activities
  • PCS – explores thoughts and feelings about pain
  • AAFP SNST – screens for social needs that may impact your health
  • EQ5D5L – measures overall quality of life and well-being
  • CFS-11 – tracks fatigue and its impact on your life
  • SMFA – checks how pain affects mobility and function
  • SWL - measures overall life satisfaction and well-being
  • QoLIBRI – focuses on quality of life after a brain injury
  • BRQ - screens for risk factors that may affect recovery

Please note: Your care team may ask you to complete other questionnaires depending on the program you are part of.

Our team uses secure systems to keep your information safe, and only your care team can see it. We do this by using:

  • Secure login – Only authorized team members can access the system.
  • Encrypted information – Your responses are scrambled when sent online and when stored, so no one else can read them.
  • Limited sharing – Your answers are only used to help your care or improve programs. They are never shared in a way that can identify you.
  • Tracking access – The system keeps a record of who views or changes information.
