Prior to submitting an agreement for review, please ensure to:

Please note that our SharePoint is separated into 2 streams: clinical and non-clinical. Your submission will fall under non-clinical unless it involves the following, in which case it should be submitted under our clinical stream:

  • Your research involves AHS facilities, personnel, and/or data, or
  • Your research project is conducted on, or for the treatment of human subjects where living human subjects, human tissues, human remains, cadavers, biological fluids, embryos, fetuses and patient information (including de-identified databases) are used.

To submit an agreement for review, you must sign into SharePoint and complete the submission form.
