Request Researcher Access to the HICCUP Database

We are pleased to be able to provide qualified research teams with contact information from families who have expressed interest and have consented to be contacted about participation in research studies through HICCUP. 

In order to request a participant contact list from the HICCUP database, you must be the principal investigator (or approved delegate) of an ethically approved, scientifically reviewed clinical research study in need of child or parent participants to serve as population samples or healthy controls.

Please complete the following form. Your request will be reviewed by the scientific director and you will be informed by email within 1 week.
