Transforming Chronic Disease Care Through Evidence, Empathy, and Equity

Learn How We’re Changing Chronic Care

About us

The Interdisciplinary Chronic Disease Collaboration (ICDC) is a team of dedicated researchers working to improve the lives of people affected by non-communicable chronic diseases—such as high blood pressure, diabetes, heart disease, and chronic kidney disease. Through collaborative research, we develop innovative health solutions that enhance care and optimize health outcomes.

Chronic conditions are the leading cause of death and disability worldwide. We recognize the challenges and complexities involved in managing one or multiple chronic conditions. Through our work, we aim to ease this burden and empower individuals to live healthier, fuller lives.

We understand that healthcare is not one-size-fits-all. Factors such as geographic location, education, and financial circumstances can significantly impact access to care and treatment outcomes.

Our research is guided by five core pillars that shape our approach and drive our impact:

  • Data science, access, and methods
  • Patient and community engagement
  • Implementation science and knowledge mobilization
  • Pragmatic clinical and health policy trials
  • Training, mentorship, and capacity building

We are committed to advancing health equity for all—regardless of age, ethnicity, gender, or background. At ICDC, we believe that opportunity, synergy, strength, and success are the foundations of impactful research. By working together across disciplines, we create meaningful change for individuals and communities living with chronic disease.
