Our team of researchers, healthcare professionals, patient partners, and research staff shares a common goal: improving the health and quality of life of Canadians living with chronic diseases.

We recognize that chronic disease is shaped by more than medical care alone. Where people live, their access to education and resources, and their financial circumstances can all influence their health. By working alongside patients, communities, healthcare providers, and research partners, we bring different perspectives together to develop evidence and advance more equitable approaches to chronic disease care across Canada.

Our work is powered by a diverse and interdisciplinary team. Researchers, project coordinators, statisticians, laboratory and data specialists, administrative and creative professionals, research assistants, trainees, and postdoctoral fellows, all contribute their expertise to our work. Together, these different skills, experiences, and perspectives allow us to ask better questions, strengthen our research, and turn evidence into meaningful improvements in chronic disease care.
