Transitions in Care for Patients with Cancer (CanTiC)

CanTiC

Transitions in Care (TiC) are the transfer of responsibility of a patient's care between healthcare providers or healthcare settings. These transitions are vulnerable periods in patient care that can lead to poor outcomes and patient experience. Patients with cancer experience many TiC from diagnosis to survivorship. The K2I Lab has explored TiC among patients with cancer and are now developing evidence-based approaches to improve TiC among patients with cancer.
