What to Expect

  1. One of the biggest differences that you may notice about the adult clinic is that you won’t see your doctor, nurse, and dietitian in one appointment. These appointments will now be separate. They will usually be on different days, and may be at different locations.

  2. Each clinic will contact you to book your first adult appointments. Please remember to call them back if you miss their call. Make sure that you only agree to appointments that you know you’ll be able to make. This is especially important for your first appointment. If you have to re-book your first appointment, you may have a long wait time for the next appointment. Some doctors may charge a fee for missed appointments.

  3. If you have to cancel your appointment, please call as soon as you know. The adult clinics are busy and the more notice you are able to give the easier it will be to reschedule.

  4. If you left ACH more than 1 year ago and you have missed your first adult appointment, you have to ask your family doctor for a new referral.

  5. You and your new team will decide how often you should see them. You may see the same person every time, or you may see a different person depending on your needs as well as the date/time of your appointment.

  6. If you don’t feel that you and your new team have a good connection, you can ask the Transition Coordinator (TC), one of your team members, or the booking clerk for a change. Also, if it’s within 1 year of your last visit to the ACH clinic, your ACH team can help you with the change as well.
