ACP GCD Discussion Toolkit

The Advance Care Planning Goals of Care Discussion Toolkit consists of senior-friendly patient information materials and healthcare provider conversation strategies, developed through conversation analysis of naturally occurring advance care planning/goals of care designation (ACP/GCD) discussions. Our research indicates that seriously ill and/or older patients display more engagement in the ACP/GCD interaction when enacted as a conversation rather than an interview, and more engagement in and understanding of ACP/GCD when patient information materials are used to help facilitate the conversation.
