We have data definitions and associated SAS code for AMI, CHF, Diabetes, Hypertension and Stroke-Related Events. We have Stata code for 29 chronic diseases; they are alcohol misuse, asthma, atrial fibrillation, lymphoma, metastatic cancers, non-metastatic cancers (breast, cervical, colorectal, lung, prostate), chronic heart failure, chronic pain, chronic pulmonary disease, chronic viral hepatitis B, cirrhosis, dementia, depression, diabetes, epilepsy, hypertension, hypothyroidism, inflammatory bowel disease, irritable bowel syndrome, multiple sclerosis, myocardial infarction, Parkinson’s disease, peptic ulcer disease, peripheral vascular disease, psoriasis, rheumatoid arthritis, schizophrenia, severe constipation, and stroke or transient ischemic attack.
