Vascular Risk Reduction

 

The program helps people manage their risk factors that can lead to a heart attack, stroke, kidney disease and/or peripheral arterial disease. Services are provided by a nurse practitioner (NP). The NP provides assessments; orders and interprets diagnostic/laboratory tests; provides referrals; monitors client progress; adjusts treatments; and provides prescriptions as needed.
