If this is your first appointment: Type 1 or Type 2 Diabetes (not pregnant)

  1. Be ready or arrive 10 minutes before your appointment. If this is a virtual appointment, please be in a quiet safe place. Do not do other tasks during the call e.g. laundry.  For your safety, we cannot do phone or zoom appointments if you are driving.
  2. Bring or have these items ready to show or read (or fill out the questionnaire a few days before coming)
    • 4 days of recent glucose readings ready to show or to read out. Include tests before all meals and bedtime. Your pharmacist can teach you how to check glucose levels and show you devices.
    • 3 days of food records to show or discuss with the dietitian.
    • A list of your medications
  3. You will find your appointments more valuable if you update your knowledge about diabetes. Please read these before your appointment and write your questions down.
  4. If you need to re-schedule or cancel, please call 403- 955-8146. Leave a message if we don't answer. Calling to cancel your appointment in advance helps us find spots for urgent patients. Thank you!

For all your appointments we will:

  • Ask about possible safety concerns.
  • Try our best to address health concerns that you have.
  • Ask to see a glucose record.
    • You may bring a written record if you like - see 'Monitoring' on our Handouts & Resources page, for examples you might want to use.
    • If you have an insulin pump or continuous glucose monitor (CGM), please upload your device before coming to your appointment. Please authorize the clinic to view your results as described in the appropriate link on this document, or bring a printout of your reports.
    • Most glucose meters allow you to upload data to a server, in which case you may keep a written record, or upload your device. Please bring a printout of your results for review with us.
