Diabetes Appointment Preparation

If this is your first appointment

Diabetes in Pregnancy
Please arrive 10 minutes early for your appointment as some Diabetes in Pregnancy clinics may ask you to fill out a questionnaire. There is no need to ‘fast’ prior to diabetes in pregnancy appointments so please have had a meal or snack before your appointment. We encourage you to view the information on www.diabetes-pregnancy.ca prior to your appointment with us. If you need to reschedule, please call the appropriate phone number listed above.

Other diabetes appointment 
Please fill out our questionnaire prior to coming. We ask that you keep a 3 day food record, so please refer to it at least 3 days before coming, if you're able to.
If you need to re-schedule, please call 403- 955-8146. We have a 24 hour answering machine if you need to leave a message. Calling us to cancel helps us accommodate urgent clients and shorten wait lists.

 

For all your appointments we will:

  • Ask about possible safety concerns.
  • Try our best to address health concerns that you have.
  • Ask to see a glucose record.
    • You may bring a written record if you like - see 'Monitoring' on our Handouts & Resources page, for examples you might want to use. 
    • If you have an insulin pump or continuous glucose monitor (CGM), please upload your device before coming to your appointment. Please bring a printout of your reports, or be prepared to log in to your account while you're here.
    • Most glucose meters allow you to upload data to a server, in which case you may keep a written record, or upload your device. Please bring a printout of your results for review with us.

 

To upload your diabetes device:

Instructions are available on the company websites. Remember that if you want to log in to view your results in clinic, you will need to have your username and password. Information for selected devices is shown below.

Carelink:

Go to CareLink Personal Login to upload your Medtronic insulin pump and access reports. While on the site you might choose to share your results with educators at Diabetes Centre Calgary. To do so, you need to enter the Clinic ID Code: 93cbe348-5425-4065-84f5-cf5657cea63c . For more information see our Carelink Patient Information Sheet

Clarity

The Clarity server is dedicated to uploading and displaying information from the Dexcom CGM. If you are using this option, please upload at home or make use of the automatic upload through the phone App. We are not able to upload to Clarity in the clinic because of the network firewall, but we are able to look at your data if you log in to your personal account on our computer.

Diasend

You may create a personal Diasend account at this link. Your account allows you to upload the Omnipod, Tandem and Ypsomed pumps and most modern meters. If you use the Dexcom CGM, you're also able to allow your Dexcom data to be transferred to the Diasend server, by inputting your Dexcom username and password from the Diasend site.

We have a contract in place to allow us to view your Diasend data, if you give us permission. You are not required to give us permission, and we do not look at your data unless you contact us to do so. If you agree to share your data with us, log in to your Diasend account, go to 'Account Admin' and input our clinic code: 91-65481.

Libre: 

LibreView is a cloud service that stores glucose data and provides reports. If using a smart phone to scan readings, data may be uploaded to the server automatically. If using the Libre scanner, you need to download a device driver to upload data from the scanner. 

  • LibreView Personal: Patients create a cloud account, upload data, email PDF reports, share data with friends/family. Note that educators are not allowed to use LibreLinkUp or to link to a  LibreView professional account. To sign up or login to LibreView access the website. 
  • AHS legal has not approved the use of LibreLInkUp or LibreView Professional for AHS clinics or educators.
