Referring Providers Information

    Diabetes Triage Criteria:

     

      All DIABETES REFERRALS FROM THE FOLLOWING ARE ACCEPTED:

      ALL DIABETES REFERRAL FROM ACUTE CARE (Urgent Care, Emergency Department, Inpatient Units, Inpatient Diabetes Navigator)

      • Referrals meeting criteria below are triaged to see a DCC educator
      • Referrals not meeting criteria below are triaged to an Inpatient Diabetes Navigator

      FMC TRANSPLANT

      • Patients with new or previous transplant and steroid induced hyperglycemia
      • Type 1 Diabetes with any of the following: 
        • Newly Transplanted
        • Previously transplanted and a) Requiring assistance with glycemic management/not achieving their individualized targets or b) Considering insulin pump therapy
      • Type 2 Diabetes with any of the following: 
        • On 2 or more antihyperglycemic agents other than insulin and not achieving their individual targets
        • On insulin and not achieving their individualized glycemic targets
        • Requiring new basal/bolus regimen start
        • Frequent Hospitalization
        • Recent hospitalization with mediation changes

      TYPE 1 DIABETES 

      • All patient referrals accepted
      • Insulin pump: Please specify if the patient is being referred for the insulin pump program.  Note that educators require an order from an MD on the  AHS Insulin Pump Therapy Program Designated MD List prior to initiation of insulin pump therapy. 

       

      TYPE 2 DIABETES with any of the following:

      • +++ Metabolically compromised requiring insulin.
      • Requiring insulin start or addition of bolus insulin or newly started on insulin in hospital regardless of current glycemic levels
      • Requiring diabetes medication adjustments secondary to steroid initiation or steroid dose changes regardless of current diabetes therapy and glycemic levels
      • On insulin or on/tried 3 diabetes agents (or less, if contraindications are noted) with any of the following:
        • Glucose readings <4 mmol/L a few times per week
        • Glucose readings frequently 12 mmol/L or higher
        • A1c 10% or higher

      • For endocrinologist diabetes, enter referral directly into Connect Care, or download Referral to Endocrinologist (note that you may specify Educator + endocrinologist on the form, if you would like both).

      Endocrinologist specialties include diabetes, hypertension, thyroid disease and thyroid cancer, dyslipidemia, fertility, and endocrine concerns.

      24 hour ambulatory blood pressure monitoring

      Blood pressure monitoring equipment is put on in clinic and then worn by the patient at home for 24 hours. For the most accurate test results we require patients to have no changes to blood pressure medications in the six weeks leading up to their 24 hour blood pressure test.
