Referrals for a diabetes educator

  • Can be sent through Connect Care by entering an order for the procedure "Ambulatory Referral to Diabetes Education".
  • Referring providers not using Connect Care can use this form. Note that you may specify Educator + endocrinologist on the form, if you would like both. Please fax outpatient (not pregnancy) referrals to 403-476-9626.

Referrals for an endocrinologist

  • Enter referral directly into Connect Care, or download Referral to Endocrinologist (note that you may specify Educator + endocrinologist on the form, if you would like both).

 

Triage Criteria

All DIABETES REFERRALS FROM THE FOLLOWING ARE ACCEPTED:

  1. All diabetes referrals from Acute Care (Urgent Care, Emergency Department, Inpatient Units)
    1. Referrals meeting criteria below are triaged to see a DCC educator
    2. Referrals not meeting criteria below are triaged to an Inpatient Diabetes Navigator
  2. Those with Type 1 Diabetes
    1. Insulin pump: Please specify if the patient is being referred for the insulin pump program.  Note that educators require an order from an MD on the  AHS Insulin Pump Therapy Program Designated MD List prior to initiation of insulin pump therapy.
  3. Those with Type 2 Diabetes with any of the following
    1. +++ Metabolically compromised requiring insulin.
    2. Requiring insulin start or addition of bolus insulin or newly started on insulin in hospital regardless of current glycemic levels
    3. New or previously transplanted patient on 2 or more antihyperglycemic agents other than insulin with and A1C > 8.5%
    4. Currently on, or have trialed 3 or more antihyperglycemic medications other than insulin with an A1C level of > 8.5 %
    5. Requiring diabetes medication adjustments secondary to steroid initiation or steroid dose changes regardless of current diabetes therapy and glycemic levels
    6. On insulin with any of the following:
      • Recent severe hypoglycemia or hypoglycemia unawareness
      • Glucose readings <4 mmol/L more than 3 times per week
      • A1c > 8.5 %  

Diabetes in Pregnancy

Those with diabetes who are pregnant or those with diabetes who are planning pregnancy. (Direct the referral to diabetes in pregnancy clinic at the hospital in which the patient will deliver.)

24 hour ambulatory blood pressure monitoring

Blood pressure monitoring equipment is put on in clinic and then worn by the patient at home for 24 hours. For the most accurate test results we require patients to have no changes to blood pressure medications in the six weeks leading up to their 24 hour blood pressure test.
