Uploading the Health History Form
- From patient chart, open "Media Manager"
- Use encounter level, by clicking on down arrow next to "Patient" and select "Choose an encounter".
- Choose the appropriate encounter [Contact Type = Appointment]
- Click "Scan"
- Document Type = "History & Physical"
- Description = "Health History Form"
- Click Scan if you have an attached scanner, or Import if the file is stored on your computer.
- Accept.
