Replacement Training Certificates
The Postgraduate Medical Education office will issue a certificate confirming completion of training for each resident at the request of the Program Director, with the approval of the Residency Training Committee and the Associate Dean PGME.
Trainees in Clinical Fellowships will be issued a certificate confirming completion of training at the request of the Program Director once they have met the training objectives as outlined in their letter of offer.
Only one certificate of completion of training will be issued by the PGME office for each period of training. Duplicate certificates will not be issued.
Replacement certificates will be issued, at cost to the trainee, should the original certificate become lost, destroyed, damaged or if a name change is required. For lost or destroyed certificates, both the replacement certificate request form and a notarized declaration must be received by the PGME office before a replacement certificate will be issued. For damaged or certificates requiring name changes, the original certificate must be returned to the PGME office with your completed request form before a replacement certificate will be issued.
