Centre for Health Informatics Room Booking Form

The Centre for Health Informatics (CHI) meeting rooms are primarily for use by staff who are housed within the centre, but the CHI Operations team makes the meeting rooms available to individuals for CHI-related matters as a courtesy. Coordination, catering, set up, and clean up of requested bookings are the responsibility of the booking party. Please note that this form is a request and may or may not be approved by the CHI Operations Team. Approval is based on availability and CHI Office Space Terms of Reference standards. Please allow for up to 48 hours for a response. Questions regarding room bookings should be directed to chi@ucalgary.ca.

The Centre for Health Informatics is accessible by key card access only. Hours of operation are from 9:00 AM – 5:00 PM. One booking attendee with key card access must be present to open the door for other attendees. The CHI door will sound an alarm if kept open, the main contact will be responsible for greeting their meeting attendees.

The room must be left in the condition that it was in when you arrived. This may require putting away chairs and replacing tables. If the event is catered, food service must be cleared, and all garbage put in the appropriate receptacles. Waste and recycling are the responsibility of the booking party. In the event that the space, equipment, or furniture are soiled or damaged, and require repairs or professional cleaning, the booking part may be charged for the necessary repair/cleaning services. By filling out this form, you acknowledge that you have read and understand the terms listed in this agreement.
