Supervisor Forms
These forms are to be completed by the CIP Research Supervisors
This document aims to ensure supervisors understand their responsibilities, provide residents with a protected way to document supervisor performance, and offer support to supervisors when areas of weakness are identified.
A completed In-Training Evaluation Report (ITER) is required twice per year for active trainees (approximately every 6 months ). It is best completed during a committee meetings.
This document reviews the CIP supervisor ToR and expectations, and is to be signed by CIP supervisors and co-supervisors.
The Final In-Training Evaluation Report (FITER) is typically completed at, or very soon after the CIP's successful thesis defence.
CIP Resident Forms
Forms to be completed by CIP residents
This document aims to ensure supervisors understand their responsibilities, provide residents with a protected way to document supervisor performance, and offer support to supervisors when areas of weakness are identified.
Use this form when you will be away for vacations, out of Calgary conferences, RCPSC exams, and research activities outside Calgary.
Request a reimbursement from CIP for course or conference expenses. Do not use this form for requests related to PGME specific travel grants.
Apply for up to $1,000 in funding to support travel to conferences or research-related activities; check the form for eligibility and instructions.
Use for maternity/parental leave, extended sick leave, bereavement, unpaid general leave (e.g. compassionate/terminal care leave). Search the PGME policy page for "Leave of Absence" for the most current form.
A brief overview of the CIP, suitable for inclusion in grant or scholarship applications, outlining the program's key features and objectives.
Committee meeting minutes are required along with your ITER. This sample provides an outline of what is expected to be included in these minutes.
