Where to Chart

Charting guidelines are intended for use by Diabetes Centre Calgary clinicians. This page is in development - ongoing updates are being made.

Charting data in a consistent place will help to

  • avoid duplicate data entry
  • enable Smart Phrases to populate the correct information
  • minimize the risk that important information is missed in subsequent encounters

Use the Search box to filter the table for the specific data element you're looking for.

Medication other than insulin   Medication Review. It gives you the option to choose "taking differently" without requesting an order. (Medication Management requires an order if changing doses). Note, if taking different doses at different times of day, enter the medication more than once (once for each time of day).
Obstetric history Gravida Para Aborta History tab > OB history. For para and aborts, do not enter numbers directly. Add previous pregnancy data. GPA updates automatically.
POCT Urine ketones, Inform II glucose Glucose goes in the Glucose table under the Diabetes Management tab; Ketones in Free Text
Preconception   Safety Checklist section of Diabetes Management tab
Preferred name   Click on Name on Storyboard, and edit
Pregravid weight Pre-pregnancy weight OB Tools tab > Prenatal Vitals and Notes > Edit. Add as Pregravid Wt.
Psycho-social   Safety Checklist section of Diabetes Management tab
Psych-social declined   Problem List > Click on Problem (diabetes) > Overview (free text psych-social services declined)
Pump Active Insulin time   In the SmartPhrases is a heading with a picklist called "additional pump programming".
Pump Back-Up Plan   In the SmartPhrase .DCCCPUMPFUORIPTPREVEW there is a picklist with an option.
Pump Brand & Model   Diabetes Management Tab > Insulin Instructions > Pump Settings (Name: free type in e.g. Settings: Medtronic 770G)
Pump - IPTP Start Date   Problem List > Advanced View > Diabetes > Amb SF Diabetes Patient Level
Pump - Omnipod "Correct Above"   In the SmartPhrases is a heading with a picklist called "additional pump programming".
Pump Start Orders    
Pump Settings   Diabetes Management Tab > Insulin Instructions > Pump Settings. You may click "pump settings" to use again to include an alternate profile if needed.
Sex, Gender   Click on Sex/gender section on Storyboard, and edit
Supplements Vitamins, Minerals, Herbs Reported medication. If you are the one recommending it, indicate that fact in the Comments box. If the desired supplement isn't there, select 'Other' under the Database tab.
Targets A1c target, ac and/or pc glucose target Problem List > Click on Problem > Overview (always use this spot, even if repeating elsewhere in the chart)
