Discharge

There have been several tickets submitted to Connect Care, to develop a native process for discharging patients. Until that is developed, providers are encouraged to use Episodes of Care to track when people are discharged from their program.

At the first patient contact with DCC, an episode of care is started.

  • Non-pregnant: Name = DCC, Type = Diabetes Chronic Disease Management
  • For DIP, use Name = DIP, Type = Pregnancy.

 

Discharge Process - non-pregnant

  • Send a communication to the physicians involved. Use the Smart Phrase DCCCDischargeForMD
  • Provide communication to the patient. Use the Smart Phrase DCCCDischargeForPt
  • Go to Episodes of Care and 'Resolve' the episode. [Note that this does not mean you are claiming that the diabetes is resolved - you are indicating that the involvement with DCC is being ended.]

Discharge Process - GDM

  • If you are not with the patient, use Encounter> Documentation
  • Once the patient has delivered, go to Episodes of Care and 'Resolve' the pregnancy episode. 
  • Send a communication to the physicians involved. Use the Smart Phrase DIPCPPEnglishForMD
  • Provide communication to the patient. Use one of the postpartum letters in the desired language.
  • Also consider episode in Soprano, if it existed there.
