Discharge: Non-DIP

When you and your patient have agreed that it’s time to discharge back to community resources:

  • Add a documentation encounter. Encounter = Documentation, Reason = Discharge from DCC
  • Send a communication to the physicians involved. Use the Smart Phrase DCCCDischargeForMD
  • Provide communication to the patient. Use the Smart Phrase DCCCDischargeForPt
  • Let Booking know that the patient is being discharged, by sending an InBasket message to “P CGY RRDTC Diabetes Educators Scheduling".
  • Booking staff will look after the rest.

Using this process, Booking staff will easily be able to distinguish discharged patients. And educators will be able to tell by looking at Chart Review > Encounters.

Discharge: Diabetes in Pregnancy

  • Add a documentation encounter: Encounter = Documentation, Reason = Discharge from DIP
  • Send a communication to the physicians involved. Use the Smart Phrase DIPCPPEnglishForMD
  • Provide communication to the patient. Use one of the postpartum letters in the desired language.

Do not resolve the pregnancy episode. Obstetrics will look after that.
