Sending Referrals/Consults

Refer DCC patient to DIP or Transfer Between DIPs

1. Transfer to Another DIP before the First Encounter

  • Use the Re-direct process:
    • Open patient on WQ 3
    • Edit Triage.
    • Change to Redirect.
    • Select the department you’re redirecting it to, e.g. “CGY FMC Endocrinology DIP”

2. Clerical Process: Transfer to a DIP Clinic after the First Encounter

  • Use “Patient Lookup” to find the patient.
  • Create an “Orders Only” encounter.
  • At the bottom of the chart, click “Add Order”.
    • Type in “Amb Ref Diab”
    • Select “Ambulatory Referral to Diabetes in Pregnancy” > Accept
    • Fill in required fields:
  • By provider = the physician who originally referred
  • To dept = CGY <site> Endocrinology DIP
  • Reason = one of the following, as appropriate
    • Preconception care
    • Gestational diabetes
    • Pregnancy and type 1 diabetes
    • Pregnancy and type 2 diabetes
  • Add comments as indicated to assist with triage.
  • Accept
  • Click Sign Orders at bottom right.
    • Order mode = Transcribed from Paper
    • Ordering provider = the physician who originally referred
    • Authorizing provider = the physician who originally referred

3. Clinician Process: Transfer to a DIP Clinic after the First Encounter

  • Open a patient encounter. If necessary, create an “Orders Only” encounter.
  • At the bottom of the chart, click “Add Order”.
    • Type in “Amb Ref Diab”
    • Select “Ambulatory Referral to Diabetes in Pregnancy” > Accept
    • Fill in required fields:
      • By provider = the educator making the referral
      • To dept = CGY <site> Endocrinology DIP
      • Reason = one of the following, as appropriate [Preconception care, Gestational diabetes, Pregnancy and type 1 diabetes, Pregnancy and type 2 diabetes]
    • Add detailed comments to assist with triage (e.g. “Type 1 Diabetes seen at DCC, now intending on getting pregnant within the next year”)
    • Accept
  • Click Sign Orders at bottom right.
    • Order mode = Per professional scope
    • Ordering provider = the educator making the referral
    • Authorizing provider = the educator making the referral (OR ‘External – Allied Health’)

To Another Connect Care Department

  • Use the "Order" box at the bottom of the patient chart.
  • No need to type in the exact name, (e.g. "ref to obstet" will work for Obstetrics) then <Enter>
  • Select 'Internal Referral (To Connect Care Department)'
  • Fill in the rest of the details.
  • When signing the order, search for and select the authorizing provider.

To an External Referral Entity

  • Use the "Order" box at the bottom of the patient chart.
  • No need to type in the exact name, (e.g. "ref to ophth" will work for Ambulatory Referral to Ophthalmology) then <Enter>
  • Select 'Outgoing Referral (To Non Connect Care Provider/Location)'
  • Fill in the rest of the details.
  • When signing the order, search for and select the authorizing provider.
  • A document is generated, which can be sent to the referral entity via RightFax.

To our Psycho-Social Team

For staff still using Soprano:

  • Continue to use the Psycho-social referral in Soprano.
  • Fax to ourselves, as usual - clerical will create the CC referral, and attach your notes.
  •  

For staff using Connect Care:

  • Click on "Add Order" at the bottom of the patient chart, type "Amb ref diab" then <Enter>. In the window that opens, select "Ambulatory Referral to Diabetes Education".
  • The following window opens:
PsychoSocial referral
  • By Provider = Yourself OR “AHS, Provider”
  • To Department Specialty = Endocrinology
  • To Department = “Calgary Zone Endocrinology CAT”
  • To Provider: leave blank
  • Reason = “Psychosocial assessment” [Pro Tip: Type “1770” then tab]
  • Add Comments as appropriate.
  • Click 'Accept'

 

  • Click 'Sign Order' at the bottom right.
    • Order Mode = Per Professional Scope
    • Ordering Provider = the provider making the request.
    • Authorizing Provider = You [OR if not allowed, “Provider-AHS” or "Provider not in system"]
