Neuro Flow Collaborative

The “Neuro Flow Collaborative” has been working together since May and will be implementing changes on four inpatient units this fall. Before this project started, we asked our partners at FMC where we could improve.

Four themes emerged:

• Team communication
• Having clear admission criteria
• Developing patient-specific care pathways
• Discharge planning

To address those four themes, the Neuro Flow Collaborative decided to develop Care Pathways, focusing on each of these inpatient Neuro units. We've engaged with partners from the Emergency Department, Diagnostic Imaging, Post-Anaesthetic Care Unit, and the Intensive Care Unit...and we have participation from managers, nursing, physicians, Allied Health and — most importantly — patients.

Our objective in creating Care Pathways for these patients is to provide efficient and high quality care to:

• reduce prolonged stays
• improve consistency in the care we provide
• enhance access to specialized services
• address capacity issues
• lower costs for everyone.

But, importantly, we will also improve the quality of care that patients are receiving and that our team is providing!
