What is required to practice in Alberta?
Alberta Practice Permit (license)
- In Alberta, all physicians practicing medicine must be licensed by the College of Physicians and Surgeons of Alberta (CPSA).

- The CPSA requires all medical credentials be source verified.

Canadian Medical Protective Association (medical liability protection)
- Alberta Health Services requires all physicians to obtain and maintain CMPA membership.
- The CMPA provides services including legal defence, risk management, educational programs and general advice.

Medical Staff Appointment with Alberta Health Services
In the process of granting clinicians access to AHS facilities and resources, Medical Affairs verifies credentials and coordinates the clinical privileging in accordance with the Alberta Health Services Medical Staff Bylaws and Rules.

What are the benefits of having a Medical Staff Appointment?
- You require a Medical Staff Appointment and clinical privileges to work in the Alberta Health Services program or facility or affiliate.
- Enabling a connection to Alberta Health Services, including obtaining an AHS email address and access to the AHS intranet.
- Collaborating with the University of Calgary and Calgary Zone Primary Care Networks (PCNs) to provide Family Medicine specific Continuing Medical Education (CME).
- Receiving communications from the DFM via bi-weekly newsletter.
- Recognizing the amazing work of local physicians with our Annual “Family Physician of the Year” and “Specialist Physician of the Year” awards.

Current Members
Contact us if you have:
- Recently changed your name, moved (home or professional address), changed email address or phone number.
- Plan to move away from Alberta or retire from clinical practice.
- Need to update your clinical privileges.
Medical Staff Appointment - Leave of Absence
Physicians with AHS privileges are required to complete an Absence from Clinical Practice Request Form for sabbaticals, extended vacations and maternity or medical leaves greater than 42 days.
- Contact fm-appt@albertahealthservices.ca to obtain the form and for any questions regarding extended leaves.
- Advantage of completing form - Physicians are not required to complete a periodic review or AIVA during a leave of absence.
- If you are a Connect Care user – you will be required to confirm if you need to maintain access during your absence from clinical practice, and if not you will need to sign out of your InBasket and your access will be removed. If you are away from your Connect Care InBasket for more than 180 days you will be required to retake your EUPA to regain access to the Connect Care system if required.
