Asthma Action Plans

Following an Asthma Action Plan will help keep
your asthma in control, even when you are
exposed to your triggers or when your asthma
symptoms are getting worse.

Do you have an Asthma Action Plan? If not, print a
plan to discuss with your health care provider.


Your Asthma Action Plan should be personalized and include:

Your Name

Contact info for your health care provider

Date the plan was created

You/your child's goals

Description of asthma control

List of daily medicines

Medicine changes for when your asthma changes

List of triggers to avoid
