Asthma Action Plans
Following an Asthma Action Plan will help keep your asthma in control, even when you are exposed to your triggers or when your asthma symptoms are getting worse.
Do you have an Asthma Action Plan?
If not, print a plan for you to discuss with your health care provider.
Your Asthma Action Plan should be personalized and include:
- your name
- the date the plan was completed
- you/your child's goals
- a list of triggers to avoid
- a description of asthma control
- daily medicines
- medicine adjustments when asthma is changing or gets worse
- contact information/numbers for your health care provider(s)
