All Nephrology referrals must be sent to our Central Access Triage. Please fax your referral to 403-592-4968.

For dialysis patients transferring to Calgary, please fill out the information checklist form and fax it to Karla Belcourt at 403-210-9881, who will then assign a Nephrologist and contact the dialysis unit.

Please note: All new patients will begin to dialyze at Foothills Medical Centre, additional sites can be requested at a later date.
